Crisis situation in modern psychiatry

In recent decades, psychopharmacology has made little progress: the number of FDA-approved drugs for the treatment of mental disorders has decreased from 13 in 1996 to only one in 2016.
Classical antidepressants are slow acting, often do not produce complete remission, and have a number of serious side effects (weight gain, sexual dysfunction, heart problems, etc.).

The beginning of the "psychedelic revolution"

In response to the deadlock of traditional medicine, interest in psychedelic substances, drugs that expand consciousness and can quickly affect the psyche, has intensified again.
After a period of active research in the 1950s and 1970s (more than 1,000 papers, 40,000 patients), in the 1970s all psychedelics were banned in the United States. The reasons were not only abuse, but also social upheaval caused by protests against the Vietnam War.

Revival of research in the XXI century

Since the early 2000s, the study of psychedelic substances has resumed, and over the past decade, their scientific research has become one of the fastest growing areas of medicine.
Today, more than 400 clinical trials are registered, and the FDA has granted breakthrough therapy status to psilocybin(depression) and MDMA (PTSD).

How psychedelic medicine works

Psychedelic substances are not drugs in the traditional sense: they are non-toxic, non-dependent, and do not have a pronounced withdrawal syndrome.
Their effect is associated with the activation of serotonin receptors (5-HT2A), which affect emotions, mood, cognition, learning, inflammation, and brain plasticity.
Therefore, they have been given a new name - psychoplastogens - drugs that can restructure pathological neural circuits even after a single dose.

The main psychedelic substances

  1. Classical ones: psilocybin, LSD, DMT, mescaline.

  2. MDMA (ecstasy): acts through serotonin and dopamine.

  3. Ketamine.dissociative anesthetic, NMDA receptor antagonist.

  4. Other substances: ibogaine, salvinorin A - with unique mechanisms of action.
